We are a family practice in the quaint rural town of Arcola, Illinois. Our office is conveniently located at 253 W Springfield Road just about one block east of Illinois Route 45. Or if driving on Interstate 57, take exit 203 and we are approximately one mile east. Cross the railroad tracks at the route 45 four-way stop light and you will find our office directly East of the 1st Mid-Illinois Bank.
We offer the latest treatments in cold laser, Activator Instrument adjusting and trigger point muscle technique. When needed to assist with healing we also have adjunctive therapies such as interferential electrical stimulation and ultrasound therapy. Dr. Middleton offers physician brand top quality nutritional supplements including Metagenics (link) products. Your well-being is important to us as we value our customers. Dr. Bob and staff are on a first name basis with our patients and want your experience with us to be as comfortable as possible, to feel welcome and at home, while still receiving professional care!
Appointments
If you would like to make an appointment, please contact our office by phone 217.208.6476 or email. If you would like to request an appointment online, please select the "Appointment Request" option from the menu. A member of our staff will follow up with you directly to confirm scheduling. Because our patients are our top priority and we understand your time is as valuable as ours, we do our best to maintain on-time schedules.
Financial
Our practice accepts cash, check, and major credit cards. We will be more than happy to submit your insurance forms for you through our electronic billing system. We do participate with BCBS and Medicare, but also bill as out-of-network physician to other insurance companies. Please call our office to verify whether your specific insurance plan is accepted. Payment plans and schedules can be discussed upon request.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours’ notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
